DESCRIPTION
The government authority in New York is seeking a vendor to provide community-based preventive services for youth ages 7–18 and their families who have been referred by the department. The primary objectives are to prevent foster care placement, strengthen family functioning, and support youth and families in achieving measurable improvements in safety, stability, and community integration.
The scope of services includes case management, home-based counseling, and interventions designed to promote safe and healthy home and community environments. Providers are expected to build the protective capacity of families, reduce behaviors or conditions that elevate placement risk, and reduce further involvement with family court or probation. Services are intended to keep youth and families safely together within their homes and communities.
