DESCRIPTION


The government authority located in Des Moines, Iowa is seeking a vendor to provide comprehensive youth centered planning services across the entire state. Services include organizing Youth Centered Planning Meetings (YCPMs) statewide, which may be accomplished by the selected vendor alone or through subcontracting with other providers. The aim is to facilitate successful reintegration or reunification for youth beginning on their first day of admission to an out-of-home placement, allowing youth and their families to identify and strengthen relationships with both formal and informal supports while developing a transition plan for return to home, community, and school.

The required services encompass comprehensive assessments to identify individual needs, strengths, and challenges for each youth, as well as the development of individualized reentry and transition plans tailored to each youth’s circumstances. Educational support must be provided upon reentry, along with robust community engagement, connecting youth to local organizations and resources including job training, recreation, and mental health programming. The vendor will also promote stakeholder collaboration among families, schools, and community organizations to establish a cohesive support network, and implement continuous monitoring and support systems to ensure ongoing progress and effectively address emerging challenges.
