Meet 10Gbps connection speed requirement
For Self-Provisioned Fiber (SPF) proposals, submit a 5% Bid Bond
Include full legal name, business address, telephone number, and legal entity type in bid
Bid must be signed by legally authorized person
Complete and submit Bid Signature Page (Appendix B)
Complete and submit Bill of Materials
Complete and submit Familial Disclosure Affidavit (Appendix D)
Provide list of at least four references, one must be a school or school district
Submit sworn and notarized Iran Linked Business Affidavit (Appendix E)
Comply with all applicable federal, state, and local codes and regulations
Pay all applicable taxes (District is exempt from Federal Excise and State Sales Taxes)
Have sufficient resources to complete Scope of Work within allotted timeframe
Not discriminate against employees
Comply with no smoking policy
Products and services must be from nationally recognized manufacturer(s)
Provide USF Service Provider Identification Number (SPIN)
Identify USF eligible products and services separately from ineligible ones
Itemize all materials, components, and equipment individually with manufacturer, part number, SKU, unit cost, and quantity
Charge Lowest Corresponding Price (LCP)
Furnish Performance Bond (for SPF projects) covering faithful performance and labor/material payment
Provide bid bond or cashier's check (5% of Bid total) with sealed Bid
Provide 100% contract amount bond with surety
Comply with Prevailing Wage for State Projects Act (Act 10 of 2023) where applicable
Provide all employees with picture ID
Employees must sign in and out at the main office
Supply weekly work schedules by Wednesday of preceding week
Submit Certificate of Insurance showing required coverage and St Johns Public School District as additional insured
Provide written guarantee/warranty for 3 years (Lit/Dark Fiber) or 7 years (SPF)
Provide installation, administration, testing, and 'As-Built' documentation
Provide test documentation on electronic media within 3 weeks of project completion
Fiber strands must be 9/125-micron, single mode, Corning SMF-28E or equivalent
Underground fiber must be installed in 1.25" outdoor grade conduit
All cables must be new from manufacturer
All cable splicing must be of fusion type
All interior cables routed through flex plenum-rated duct
All interior routes 50' or longer require transition to plenum-rated interior cable
Install fire stop system in accordance with specifications and manufacturer's recommendations
Provide engineering and permitting for constructing routes
Include best estimate of make-ready fees and permitting costs in the Bid
Provide an overview of maintenance practices including routine maintenance, network monitoring, scheduled maintenance windows, handling unscheduled outages, and contacting district representative before dig location services
Provide and install identification tags for all exterior cables
Develop and submit a labeling system for cable installation for approval
