Seeking providers of vocational rehabilitation services for individuals with mental or physical disabilities in Washington, DC.

Submit signed HCA application and any issued amendments.
Submit Contractor Qualification Record with credential attachments, completely filled out, with particular attention to financial responsibility, disclosure, organization history, background, experience, education, credentials, licensure, personnel critical to organization performance, certifications, and incorporations.
Submit Articles of Incorporation or Articles of Organization.
Submit a brief program description for each service applying to provide, either as a Word/PDF document, a link to the company's website, or a detailed background description if no website.
Submit completed EEO forms.
Submit completed and signed First Source Agreement.
Submit completed and page-bottom initialed First Source - Revised Employment Plan.
Submit responsibility documents (see Section L.6.1).
Submit completed and signed Bidder/Offeror Certification Form.
Submit Master Supplier Form.
Submit Certificate of Liability Insurance, attesting that coverages and limits are compliant with Section I.8, including Commercial General, Automobile Liability, Workers' Compensation, Employer's Liability, Cyber Liability, Professional Liability, Sexual/Physical Abuse & Molestation, and Commercial Umbrella or Excess Liability.
Ensure insurance policies contain required language, including a waiver of subrogation provision, additional insured status, and 'Government of the District of Columbia' as the Certificate Holder.
Submit Certificate of Clean Hands, dated within 30 days.
Submit Business License.
Read and acknowledge that being awarded an HCA requires procuring and maintaining the required insurance coverages and limits specified in Section I.8.
Ensure all applicable parts of required forms are completed legibly and in their entirety.
Maintain complete written job descriptions for all positions within the program, including education, experience, licensing/certification, duties, responsibilities, hours, salary, background check, and performance evaluation criteria.
Obtain and document written work experience and personal references for staff.
Ensure all specialists, employees, and subcontractors serving persons 17 years or younger complete a background clearance check (FBI, Child Protection Registry, Jurisdictional Police Department Check).
Have an accessibility evaluation with quality assurance of the designated office, including wheelchair accessibility and ADA requirements for halls, office, stairs, restrooms, and doors.
Complete HCA orientation and progress report training from the District prior to rendering approved services.
Provide and maintain a current organizational chart displaying administrative oversight and clinical supervision responsibilities.
Describe the frequency, duration, and methods of supervisory oversight of staff.
Notify contracting administrators in writing of any changes in staffing patterns, job descriptions, or personnel within ten business days.
Notify the Contract Administrator immediately in writing of any exiting employees.
Submit documentation that all direct and indirect staff, including consultants, are free of communicable diseases and meet criminal background check requirements.
Maintain an individual personnel file for each staff person.
Maintain documentation that each staff person possesses adequate training and meets all applicable certification/licensing requirements.
Conduct orientation sessions for all staff members regarding administrative procedures, program goals, policies, and practices.
Attend all orientation sessions provided by the District for staff members.
Deliver culturally competent services.
Deliver linguistically competent services, including for deaf, hard of hearing, deaf-blind, visually impaired, blind, Spanish speaking, and other languages.
Ensure the service environment offers wheelchair, public transportation, language, and other accessibility features.
Ensure office and building locations comply with fire and building safety codes.
Maintain documentation of compliance with service location requirements.
Provide sufficient qualified staff, approved by the Contract Administrator.
Ensure staff positions meet specific qualifications for Program Manager/Coordinator, Work Adjustment Trainer/Coordinator, Vocational Trainer/Instructor, Employment/Placement Specialist, Job Coach, Benefits Planner, and Support Service Provider.
Disclose staff qualification and credentials to the Contract Administrator for review and approval.
Maintain evidence of completion of a professional development course (Basic Employment Certificate or ACRE comparable, 40 hours of specific topics, or National Certificate in Employment Services/Certified Employment Support Professional).
Ensure personnel have a 21st-century understanding of the evolving labor force and needs of persons with disabilities, with specialized training and experience.
Certify compliance with all applicable tax requirements of the District of Columbia.
Comply with the Nonprofit Fair Compensation Act of 2020 (if applicable).
Comply with the 51% District Residents New Hires Requirements and First Source Employment Agreement.
Comply with the Protecting Pregnant Workers Fairness Act of 2016.
Comply with the Unemployed Anti-Discrimination Act of 2012.
Comply with the Fair Criminal Record Screening Amendment Act of 2014.
Comply with the Non-Discrimination Clause.
Submit electronic copy of proposal, redacted.

Provide a written narrative of the service provider’s vocational rehabilitation service and professional references, reflecting a track record of obtaining state and federal funding, previous outreach activities, knowledge/skills for Rehabilitation Act purposes, and history of working with individuals with disabilities.

The contract term is for one year from the date of award, with the possibility of four one-year option periods, not to exceed a total of five years.
