Underground Utility Locating Services

The City of Dover, Delaware is seeking a contractor to provide complete underground utility locating services, including mapping in digital and paper formats.

Submit three paper copies and one electronic copy (CD/DVD or flash drive) in a sealed envelope
Email or fax RFP notice to receive published addenda if intending to submit a proposal
Ensure submission is not revocable for 90 days
Comply with the Underground Utility Damage Prevention and Safety Act
Provide complete personnel, office services, vehicles, tools, and materials
Ensure locators have cell phones and provide a contact number staffed during normal business hours
Be responsible for locating unmapped facilities
Respond to emergency ticket requests within two hours
Properly connect to infrastructure for accurate locating
Operate during normal business hours (Monday-Friday, 7:00 a.m. to 3:30 p.m.)
Receive and return tickets within mandated times (98% on time, 100% responded to)
Be financially responsible for damages due to mismarking or failure to mark
Maintain confidentiality of City plans, maps, and documentation
Include primary contact information, experience, proof of insurance, and exceptions in the proposal
Keep records for up to six years, including ticket details, facility types, and site photographs
Notify the City of unlocatable facilities and await direction before clearing tickets
Comply with all applicable Delaware Department of Transportation (DelDOT) requirements
Allow City to vet and refuse new employees
Notify the City immediately of any damage to City-owned underground infrastructure
Be authorized to do business in the State of Delaware and obtain a City of Dover Business license
Provide a brief history of the firm (limit two pages)
Describe services and explain how they meet the scope of work
List at-fault damages and violations over the past five years
Provide three references from Delaware entities for services provided in the past three years
Include a staff listing with experience within the industry and current company
Provide documentation of a current safety plan/policy and periodic safety training
Disclose any litigation or administrative proceedings where the bidder was ruled against in the past three years
Provide a statement of no conflict of interest
Complete, sign, and return the 'Consent for Disclosure Under the Freedom of Information Act (FOIA)' form
Clearly mark any proprietary information within the bid submission
Identify qualification and claim for local or minority vendor preference on proposal documents
Provide authoritative proof of minority ownership (e.g., certification directory)
Provide references to the City of Dover upon request
Maintain Worker's Compensation and Employers' Liability Insurance with a limit of liability no less than $1,000,000, including a waiver of subrogation in favor of the City of Dover
Maintain Commercial General Liability Insurance with limits of liability no less than $1,000,000 general aggregate, $1,000,000 products/completed operations aggregate, $1,000,000 personal injury, and $2,000,000 each occurrence
Maintain Automobile Liability Insurance with limits of liability no less than $500,000 per person, $1,000,000 each occurrence, and $1,000,000 aggregate
Ensure insurer is authorized to do business under the laws of the State of Delaware
Furnish a Certificate of Insurance with the City of Dover named as an additional insured
Provide thirty calendar days advance notice of cancellation or nonrenewal of insurance
Maintain Contractors Professional or Errors & Omissions Liability Coverage with limits of $2,000,000
Maintain Commercial Umbrella Liability with Limits of $10,000,000 Occurrence Limit
Maintain Privacy (Cyber) Liability with minimum Limits of $1,000,000
Provide a Waiver of Subrogation in favor of the City of Dover
Comply with all applicable codes, laws, regulations, standards, and ordinances
Obtain and pay for all required permits and licenses
Maintain accounting records for three years after invoice date
Warrant that no contingent fees have been paid or agreed to be paid
