Seeks provider for comprehensive transitional living group homes and apartments, including supportive services, for government needs in Jefferson, Missouri.

Have a minimum of three years' experience working with the Child Welfare system in the State of Missouri.
Have a minimum of three years' experience in assisting youth with the development of life skills accomplishments.
Ensure at least one Executive and Professional Team member has a minimum of three years of leadership experience, three years' experience with the Missouri Child Welfare system, and three years' experience in life skills development.
Ensure Direct Care Team members have a minimum of one year's experience with the Missouri Child Welfare system and one year's experience in life skills development.
Ensure all personnel possess required Missouri licenses and certifications.
Conduct Family Care Safety Registry (FCSR) background investigations for all direct service personnel annually.
Submit fingerprints for criminal background checks (Missouri State Highway Patrol and FBI) for all personnel.
Conduct background investigations in other states for personnel residing or relocating from those states, annually if they continue to reside out-of-state.
Prohibit personnel with certain felony/misdemeanor convictions or substantiated child abuse/neglect findings from providing services.
Disclose any relevant litigation, pending investigations, assessments, or substantiated findings involving the contractor, employees, officers, agents, or subcontractors.
Be fully operational within 45 calendar days after state agency authorization to proceed with services.
Ensure all facilities meet local, city, county, and state building ordinances.
For Pre-Transitional Living Group Homes (PTLG) and Transitional Living Group Homes (TLG), possess a valid, current residential care license.
Develop and display emergency procedures and guidelines, including evacuation plans, within each living arrangement.
Place a disaster supply kit in an easily accessible location within each apartment.
Assist youth with establishing and maintaining a positive credit score.
Provide maintenance services including food, clothing, shelter, transportation, and daily supervision/oversight.
Not become the payee for any monies received by the youth (e.g., Supplemental Security Income, Old Age Survivors & Disability Insurance).
Maintain a savings account for each youth, depositing funds received for bills and providing monthly accounting summaries.
Provide monetary support to youth for activities enabling transition to independence and social/educational needs.
Provide services to youth to aid in the development of permanent connections, utilizing the Permanency Pact Agreement.
Provide adjunct services including education, vocational, religious, recreational, and socialization experiences.
Arrange medical, dental, and mental health care services through the MO HealthNet program.
Provide transportation for all youth to appointments and activities.
Develop an Individualized Day-to-Day Service Plan within 30 calendar days after a youth is placed.
Provide Chafee Foster Care Program (CFCP) services to 100% of all referred youth, with specific percentages in identified service elements.
For PTLG services, serve youth aged 15.5 through 18 with a goal of transferring to a Transitional Living Placement.
Adhere to federal National School Lunch Program (NSLP) guidelines for food in PTLG and TLG facilities.
For TLG services, provide a minimum of two progressive stages of autonomy services on the same campus.
For Transitional Living Single/Scattered Site Apartments (TLSS), ensure apartments are located within Missouri, provide easy access to community resources, and maintain a pleasant, clean, safe, and healthy environment.
For TLSS, provide proof of efforts to confirm crime-free areas for apartment locations.
For TLSS, have a pest and rodent control inspection and fumigation plan.
For Structured TLSS, provide a support person accessible 24 hours per day, seven days per week, with visits at least every 72 hours.
For Unstructured TLSS, be available to the youth upon request and visit a minimum of once per week.
Ensure youth eligible for TLP services are referred by the state agency and have a case plan of Another Planned Permanent Living Arrangement (APPLA), except for PTLG.
Complete an entry screening/interview within ten calendar days after receipt of referral.
Provide a written response (acceptance/rejection) within five calendar days after the entry screening/interview.
Complete an initial assessment within 30 calendar days after a youth is placed.
Submit the Individual Life Skills Progress form (CD95) on a monthly basis.
Participate jointly with the Family Support Team and state agency in determining youth's discharge.
Provide an exit packet to youth aged 17.5 and older, including resources for transitioning from care.
Use only state agency approved equipment to remotely connect to the state agency’s network and utilize a secure virtual desktop environment (VDI).
Update the state automated system (FACES) accurately and in a timely manner.
Submit monthly progress reports (CD95) and NYTD Older Youth Services and Financial Expenditure reports in FACES.
Submit a quarterly outcome report in FACES by the 15th of the month following the quarter ending.
Submit an annual written report for the previous calendar year by January 31st of each year.
Submit oral and written incident reports within specified timeframes for unusual and critical incidents.
Provide a quarterly service plan detailing the day-to-day service plan for each youth.
Obtain prior state agency approval on all forms utilized.
Participate in training opportunities provided by the state agency.
Cooperate with the local education authority to facilitate timely transfer of school records.
Notify the state agency in writing 30 calendar days in advance of any changes in the TLP affecting service delivery.
Allow reasonable and timely site visits by the state agency.
Maintain confidentiality of all data and information accessed under the contract.
Comply with all applicable federal and state laws and regulations, including HIPAA, Drug Free Workplace Act, Pro-Children Act, Whistleblower Protections, Human Rights and Affirmative Action, Clean Air Act, and Federal Water Pollution Control Act.
Be registered in MissouriBUYS.
Be in tax compliance with the Missouri Department of Revenue and provide a 'Vendor No Tax Due' certificate.
Be properly registered with the Missouri Secretary of State.
Enroll and participate in the E-Verify federal work authorization program (if a business entity).
Certify non-boycott of goods or services from the State of Israel (if applicable).
Disclose if any services are performed at sites outside the United States.
Disclose any employee/conflict of interest.
Provide Federal Funding Unique Identity ID.
