Lubbock, Texas seeks providers for student athletic and catastrophic insurance covering a variety of extracurricular and vocational activities for one year.

Provide insurance proposals for UIL activities, athletics, JROTC, vocational education, Future Farmers of America, cheerleaders, drill teams, extra-curricular, and voluntary student accident insurance.
Proposals must closely match District specifications.
All items on the Specification Questionnaire must be answered.
The policy issued must duplicate the answers on the Specification Questionnaire.
The company or agent must warrant that the policy will be amended to comply with District specifications or be responsible for providing benefits as described.
The company awarded the UIL/Athletic insurance contract will also receive the Student Voluntary Insurance contract.
Catastrophic and basic insurance carriers must have an A.M. Best rating of at least A-(VII).
Offer 24-Hour Coverage up to $30,000 with a $500 deductible as a primary policy, accepted on full assignment through a PPO or special network group.
Offer At School Coverage up to $30,000 with a $500 deductible for injuries at school or school-sponsored activities (excluding varsity football) as a primary policy, accepted on full assignment through a PPO or special network group.
Offer Voluntary Football Coverage (Varsity) up to $30,000 with benefits matching the at-school plan.
Offer Dental Coverage paying Usual and Customary charges up to $1,000 for injuries to sound teeth.
Offer Blanket Catastrophic Accident Insurance for UIL sports and activities with a $25,000 deductible and an indemnity amount of no less than $1,000,000, with a 2-year deductible period and a 10-year minimum benefit period.
Offer Blanket Coverage for All Interscholastic Activities (grades 6-12) up to $30,000 with a $500 deductible, as an excess policy, effective August 1, 2026, and expiring July 31, 2027. Benefits must be accepted on full assignment through a PPO or special network group and include Vocational classes, ROTC, and FFA.
