Perform and submit an Organizational Conflict of Interest (OCI) analysis with Phase III proposal
Have a current NIST SP 800-171 DoD assessment posted in the Supplier Performance Risk System (SPRS)
Comply with Attachments 1–6 of this CSO for all submissions
Mark each page of submissions containing proprietary information
Disclose any actual or potential Organizational Conflict of Interest (COI) early in the process
Submit Phase III Price Proposal with detailed pricing information in an editable Microsoft Excel format
Excel pricing model must contain all cost elements, show formulas, identify base and option periods, include labor category details, material/equipment lists, travel costs with assumptions, and subcontractor costs
Phase III Price Proposal must include a Summary Pricing Table, Basis of Estimate (BOE), Labor and Material Breakdown, Subcontractor/Partner Pricing, and Supporting Market Data
Submit a standalone, contract-ready Performance Work Statement (PWS) excluding proprietary information
PWS must define objective, scope, tasks, technical requirements, standards, constraints, and additional requirements
Phase I PowerPoint briefing must be in Microsoft PowerPoint (.pptx) format, maximum five content slides plus title slide, not exceed 10 MB, and use Times New Roman font size ≥ 12 pt
Phase I White Paper must be 8.5 x 11 inches, one-inch margins, maximum two narrative pages plus cover page, single-spaced, Times New Roman font size ≥ 12 pt, submitted as a PDF not exceeding 10 MB
Phase I White Paper must align with the Area of Interest (AOI) and expand upon PowerPoint/Quad Chart elements
Do not include corporate or personnel qualifications, past performance information, or unrequested supplemental material in Phase I submissions
Clearly mark file names with CSO#, AOI#, and Company Name
Send a separate email (without attachments) to the Contracting Team after emailing a submission for receipt confirmation
