A government authority in Kansas is seeking a vendor to provide self-pay permit printing services. The required permits must be produced as a continuous three-part carbonless form. Parts 2 and 3 must be constructed and glued to form an integrated envelope, with all parts properly aligned and collated in sequence.
The forms should measure 7.375 inches wide by 3.75 inches high, with a horizontal perforation 0.5 inches from the top to create a stub tear-off, and a vertical perforation dividing the form into two equal sections. Transfer adhesive tape is to be applied on the face below the horizontal perforation to facilitate sealing. There are additional requirements for adhesive tape placement, including along the bottom edge to seal the envelope, and for ensuring adhesives do not affect legibility or imaging. All inquiries must be submitted by May 15, 2026.
