The selected vendor will be responsible for assisting organizations in establishing and expanding rural community health worker programs, providing technical support to facilitate long-term financial sustainability through guidance in Medicaid claims, Medicare billing pathways, and integration into organizational structures for durable funding and reimbursement. The vendor will work to strengthen partnerships with insurers, educate payers on reimbursement models, and support greater coverage of services.
Additional responsibilities include providing monthly progress reports, an annual site report, and other reports as needed. The vendor will offer technical assistance, education, and training related to program roles and responsibilities, chronic disease management, community engagement, Medicaid, and other topics as requested by healthcare providers and facilities. Knowledge of social and non-medical determinants of health—such as housing, transportation, food access, and recreation—is required.
Effective communication regarding programs, planning, and policy implementation to a diverse range of stakeholders is expected. The contract period will be one year. A letter of intent and questions must be submitted by specified deadlines prior to proposal submission.
