DESCRIPTION
The government authority located in New York is seeking a vendor to provide re-entry transitional management services for individuals being released from county jail. The selected provider will be responsible for delivering a comprehensive continuum of transition management and re-entry support to facilitate participants' long-term wellness and recovery.
