Seeking a vendor for the supply of various promotional items including apparel, accessories, and awards in Georgia.

Comply with Byrd Anti-Lobbying Amendment for orders exceeding $100,000.
Certify regarding debarment, suspension, proposed debarment, and other responsibility matters for orders exceeding $30,000.
Comply with Georgia Security Immigration E-Verify requirements.
Certify not to be a scrutinized company involved in Sudan business operations.
Comply with Georgia Open Records Act regarding trade secrets.
Certify no conflict of interest with Kennesaw State University employees or officials.
Agree to not use Kennesaw State University's name or logo without prior written consent.
Accept all RFQ specifications and Kennesaw State University's terms and conditions.
Obtain prior written consent from Kennesaw State University for any subcontracting or assignment.
Possess administrative and fiscal capability to provide promotional items.
Submit all required documents listed in the Checklist of Required Documents.
Provide business licenses for the last three years of operating history.
Comply with all applicable Federal, State, and local laws and regulations.
All shipping, artwork, setup fees, rush orders, screen printing, and embroidery costs must be included in the price.
Agree to replace or pay for units damaged or lost while in the contractor's possession.
Notify Kennesaw State University of any circumstances that may delay item delivery.
Obtain written approval from Kennesaw State University for all additional work or change orders.
Adhere to communication restrictions with Kennesaw State University staff during the solicitation period.
Provide three to five references from higher education customers for similar projects within the last two years.
Accept purchase order, payment request (invoice), and Procurement Card payment methods.
Complete all work in a timely manner as outlined on the order.
Not require a minimum quantity or dollar amount for an order.
Provide information on return policy for erroneously ordered items, with no restocking fee for vendor errors.
Fulfill orders as per the attached cost worksheet.
Describe steps to determine if digital service or technology solutions support inclusive access.
Describe steps to validate accessibility of design proposals with potential disabled users.
Describe how accessibility requirements are incorporated into acquisition and development activities.
Agree to price match the lowest vendor's price if identified as a Small Business or Georgia Resident Small Business (within 5% and $10,000 of the lowest bid).
Obtain Kennesaw State University Trademarks licensee approval prior to contract execution, if not already approved.
Maintain specified insurance coverage (Workers Compensation, Commercial General Liability, Automobile Liability, Umbrella Liability) with the State of Georgia as an additional insured.
Provide a performance bond for projects at or greater than $100,000.
Provide a payment bond for projects at or greater than $100,000 if subcontractors are utilized.
Ensure employees undergo security background checks for roles involving 'regular interaction' with staff, faculty, students, minors, or sensitive data.
Ensure employees maintain professional conduct and appearance.
Ensure employees wear contractor's picture ID badges.
Comply with University System of Georgia Tobacco and Smoke-Free Campus Policy.
Adhere to Kennesaw State University parking regulations and purchase parking passes.
Certify compliance with ANTI-BDS (Boycott, Divestment, and Sanctions Campaign Against Israel) legislation (O.C.G.A. 50-5-85).
Comply with FERPA regarding education records.
Comply with Board of Regents of the University System of Georgia travel policy for reimbursement.
Adhere to University System of Georgia Ethics and Title IX policy.
Comply with Clery Act / Campus Security (House Bill 280).
Complete and return all necessary supplier setup forms, including e-verify, supplier data sheet, and W-9.
Bid on the unit price for each category in the Sourcing Event.
Include all applicable shipping costs for FOB Destination.
Complete all Mandatory Questions.
