Ensure subcontractors for subcontracts exceeding $25,000 certify non-debarment/suspension
Disclose any communications with CapMetro employees or representatives outside approved channels
Disclose if any company or person was employed/retained to solicit the contract on a contingent fee basis
Comply with CapMetro's Code of Ethics
If requested, submit a 'Certificate of Interested Parties' with the Texas Ethics Commission
Provide workers' compensation insurance coverage for all employees on the Project
Certify that the Contractor does not boycott Israel
Certify that the Contractor is not engaged in business with Iran, Sudan, or a foreign terrorist organization
Verify that the Contractor does not discriminate against firearm entities or trade associations
Verify that the Contractor does not boycott energy companies
Certify that the Contractor is not owned/controlled by citizens/governments of China, Iran, North Korea, Russia, or a Governor-designated country if the contract relates to critical infrastructure
Prime Contractor must perform no less than 30% of the work with its own organization
Disclose if providing or using covered telecommunications equipment or services from prohibited entities
Submit pricing for all line items in Exhibit A
Sign and date each page of the pricing schedule
Do not include any exceptions to solicitation terms, conditions, or requirements
Furnish proof of CapMetro-stipulated insurance requirements
Contractor and subcontractors' employees must wear ANSI Class 2 or 3 high-visibility safety vests on CapMetro premises
