Certify compliance with Michigan Public Act No. 517 of 2012 regarding Non-Iran Linked Business.
Adhere to the Non-Discrimination Clause as per Title VI of the Civil Rights Act of 1964, Elliot Larsen Civil Rights Act, and Michigan Handicapper's Civil Rights Act.
Provide Worker's Compensation Insurance.
Provide Commercial General Liability Insurance with limits not less than $1,000,000 per occurrence and aggregate, including Contractual Liability, Products and Completed Operations, Independent Contractors Coverage, and Broad Form General Liability Extensions.
Provide Automobile Liability Insurance with limits not less than $1,000,000 per occurrence, combined single limit for Bodily Injury and Property Damage, covering all owned, non-owned, and hired vehicles.
Provide Owners' and Contractors' Protective Liability with limits not less than $1,000,000 per occurrence and aggregate.
Name the City of Madison Heights, all elected and appointed officials, employees, volunteers, boards, commissions, and authorities as additional insureds on Commercial General Liability and Motor Vehicle Liability policies.
All policies must include an endorsement for 30-day advance written notice of cancellation (10 days for non-payment of premium) to the City of Madison Heights Purchasing Department.
Provide a Certificate of Insurance and required endorsements at the time of contract execution.
Deliver renewal certificates and endorsements at least ten days prior to the expiration date of any coverage.
Print and mail the Madison Heights Happenings Recreation Guide.
Mail the guide four times per calendar year (February, May, August, November) to approximately 13,377 Madison Heights residential homes per mailing.
Offer two options for guide specifications: Option 1 (24-36 pages, 8.5" x 11", full color, saddle stitch, #50 uncoated offset paper 92 bright) or Option 2 (24-36 pages, 8.5" x 11", full color, Cover: 4-page, 70 lb. Gloss Text #3 or better; Inside: 50 lb. Offset / 92 bright, saddle stitched).
Provide a 2% overrun of brochures, delivered to the Department of Public Services at 801 Ajax Drive, Madison Heights, MI 48071, with no underruns accepted.
Be responsible for the entire printing, packaging, and mailing process, including obtaining/updating the mailing database and processing for United States Postal Service delivery.
Accept the final product as a PDF in InDesign format and provide a method for electronic file sharing.
Allow the City a minimum of 24 hours to inspect, proofread, and return proofs.
Provide a seven-workday turnaround from vendor receipt of the recreation guide to delivery of the final printed product to the United States Postal Service.
Notify the City of any delays in the schedule within 24 hours.
Ensure high and consistent product quality, adhere to deadlines, and avoid issues such as grey/washed out halftones, improper registration, inconsistent ink coverage, or paper folds.
Do not change paper or make type changes without prior written approval from the City.
Provide a total, all-encompassing bid amount.
Provide the name, telephone number, and email of the sales representative or production manager assigned to the contract.
Disclose any subcontractors used for any portion of the printing and/or binding.
Complete the Qualification Questionnaire.
Complete the Non-Collusion Affidavit.

Initial contract shall be for a two-year period from date of Council Approval to include guide production through and including the publication for May 2028.
