Disclose any felony convictions, guilty pleas, or nolo contendere for the Respondent or its personnel.
Disclose any bankruptcy or insolvency proceedings in the last ten years.
Disclose any material, pending litigation that could affect contract performance or financial condition.
Disclose any pending or in-progress Securities Exchange Commission investigations.
Provide a descriptive statement detailing evidence of the Respondent's ability to deliver the goods or services.
Provide a narrative description of the proposed contract team, organizational structure, and key personnel with resumes.
State whether subcontractors will be used, including their names, contact information, scope of work, and assent.
Disclose any current or completed contracts with the State of Tennessee within the previous five-year period.
Disclose if the Respondent is presently debarred, suspended, or excluded from covered transactions.
Disclose any convictions or civil judgments for fraud, criminal offenses, or related acts within the past three years.
Disclose any indictments or criminal/civil charges for specified offenses.
Disclose any public transactions terminated for cause or default within the past three years.
Provide three references from different persons in accordance with Attachment F.

Demonstrate creativity and execution by providing an example plan with renderings and photographs from a similar project.
Provide a narrative illustrating experience and capabilities in interpretive exhibition fabrication and design.
Provide a narrative detailing experience with universal accessibility and examples of ensuring universally accessible exhibits.
Comply with all provisions and requirements of the RFQ and any resulting contract.
Comply with applicable Tennessee laws, Title VI, Title IX, Equal Employment Opportunity Act, and Americans with Disabilities Act.
Ensure the response information is accurate and independently prepared.
Affirm no direct or indirect payments to State employees or officials in exchange for contract-related actions.
Ensure the response remains valid for at least 120 days.
Certify compliance with the Iran Divestment Act.
Do not knowingly utilize the services of an illegal immigrant or subcontractors who utilize illegal immigrants.
Maintain Commercial General Liability (CGL) insurance with single limits not less than $1,000,000 per occurrence.
Maintain Workers' Compensation and Employer Liability insurance as required by state statutes or provide proof of exemption.
Maintain Professional Liability insurance with a limit not less than $1,000,000 per claim and $2,000,000 in the aggregate (or $3,000,000 for medical professionals).
Maintain Automobile Liability insurance with a limit not less than $1,000,000 per occurrence or combined single limit.
Provide a Certificate of Insurance (COI) naming the State as an additional insured (except for Workers' Compensation and Professional Liability).
Ensure all policies contain an endorsement for a waiver of subrogation in favor of the State.
Obtain State approval for any deductible or self-insured retention over $50,000.

Phase 2 content deliverables will require the development of an exhibit outline that weaves the content into a cohesive narrative, incorporating object labels, image captions, and credits.
