A provider is sought to deliver parent support outreach and family community partnership services to at-risk families in Minnesota. The program focuses on family strengths and specific needs, with an emphasis on serving families facing challenges such as homelessness, financial hardship, and parenting concerns. Services will include comprehensive assessments by program workers to identify strengths and areas where families may need support.

Selected vendors will offer case management and direct assistance aimed at meeting basic needs such as housing, transportation, and childcare, all with the objective of promoting child safety and wellbeing. Eligible families include those identified through screened-out child maltreatment reports, community referrals, or self-referrals, with at least one child aged 17 or younger or who are currently pregnant. Questions about the program must be submitted by December 29, 2025.
