RFP seeking qualified firms to provide nutrition services for a government authority in Memphis, Tennessee.

Provide all required state licenses and certifications.
Possess an active Equal Opportunity Compliance BRN and Vendor number.
Adhere to Title VI requirements.
Comply with the Tennessee Lawful Employment Act (E-Verify).
Submit a completed, signed, and notarized Drug-Free Workplace Affidavit.
Have a minimum of five years of experience in providing similar services.
Submit organizational history, governing body details, organizational chart, W-9, mission & values, personnel information (supervisory structure, qualifications, training), audited financial statements, business status, certificate of insurance, workers compensation insurance, bank reference, two credit references, organizational conduct, internal monitoring tool and procedures.
For current ACMS vendors, submit accurate and timely billing and data reporting records; for non-current vendors, submit similar records from a current contractor.
Complete and submit Attachment 1, including service delivery explanation, implementation timeline, background check policy, and holiday schedule.
Provide customer satisfaction survey results demonstrating 80% or more satisfied customers.
Submit a copy of the most recent congregate meal site agreements or a site agreement template.
Demonstrate ability to provide services in Fayette, Lauderdale, Shelby, and Tipton counties.
Ensure proposed service costs are equal to or less than the maximum allowable cost.
Comply with DDA Aging and Disability Program & Policy Manual guidelines for authorization, service description, eligibility, gratuities, confidentiality, and general compliance.
Adhere to Older Americans Act Sections 306 and 307 regarding targeting populations.
Provide services five days per week.
Ensure sufficient staff, including a Nutrition Director with current food safety certification.
Utilize advice and expertise of a dietician or other qualified individual.
Solicit voluntary contributions and not supplant funds.
Have plans for weather-related emergencies.
Assist participants in taking advantage of other benefits.
Coordinate services to avoid duplication.
Be licensed in accordance with state regulations.
Provide services consistent with Provider Authorization.
Notify Case Manager of changes in individual's condition within five business days.
Implement data collection and reporting procedures.
Maintain current Food Service Establishment Inspection Reports.
Maintain safe and appropriate meal temperatures during transit.
Provide nutrition education, outreach, and screening.
Monitor kitchens, congregate meal sites, and home delivered meal programs.
Submit a plan of correction for monitoring findings.
Assure paid or voluntary staff meet minimum qualifications and training standards.
Designate a supervisor for service delivery.
Procure and maintain liability and workers' compensation insurance.
Obtain sufficient bond coverage.
Comply with Tennessee Code Annotated § 52-2-1002 on background checks.
Implement an ongoing Quality Assurance process.
Obtain written approval from AAAD for meal site changes.
Develop complaint and grievance procedures.
Provide a signed written request for funds as specified in the contract.
Notify Adult Protective Services and AAAD of unsafe conditions or suspicions of abuse/neglect.
Develop denial or termination of service procedures.
Develop and implement a policy manual covering fiscal management, food service management, safety and sanitation, staff responsibilities, and organizational chart.
Be incorporated under the laws of the state of principal business.
