Attend a mandatory pre-bid conference
Provide a detailed estimate for cost changes due to contemplated changes in Scope of Work
Notify the City of any estimated change in the completion date due to contemplated changes in Scope of Work
Advise the City if contemplated changes affect the Contractor's ability to meet completion dates or schedules
Suspend work on affected Scope of Work portions if instructed by the City
Not commence work on changes until a written amendment is signed by the authorized representative
Maintain adequate records for at least three years after contract completion for audit purposes
Treat all employees equally without discrimination based on race, color, religion, disability, sex, age, national origin, ancestry, marital status, or sexual orientation
Maintain all required licenses and approvals to conduct business
Deliver all documents and materials prepared for the City in hard copy and electronic mail for final payment
Keep confidential all non-public information obtained or developed by the City
Comply with Chapter 119, Florida Statutes (Public Records Law)
Maintain public records required to perform services
Provide copies of requested public records to the City upon request
Ensure exempt or confidential public records are not disclosed except as authorized by law
Transfer all public records to the City upon contract completion or maintain them according to Applicable Law
Notify the City Representative immediately upon receiving a public records request
Submit any material claimed as 'RESTRICTED MATERIAL' with a sworn declaration and specific statutory exemption
Maintain minimum insurance coverages as specified (Professional Liability, Comprehensive General Liability, Comprehensive Automobile Liability, Workers' Compensation)
Furnish Certificates of Insurance to the City's Representative prior to commencing operations
Ensure no material change or cancellation of insurance without thirty days prior written notice to the City's Representative
Include the City of Lauderdale Lakes as 'Additional Insured' on all insurance policies except Professional Liability and Workers' Compensation
Submit new or renewed insurance certificates at least thirty calendar days prior to expiration
Demonstrate successful completion of services as specified in the Technical Specifications / Scope of Services for a minimum of three years
Be normally and routinely engaged in performing such services for a minimum of three years
Be properly and legally licensed to perform such work
Have no conflict of interest with regard to any other work performed for the City
Provide a statement summarizing understanding of the work and commitment to perform
Provide an outline of the elements and organizational structure of the project management team
Describe the Contractor's approach to organizational management and staff responsibilities
State experience in providing Printing and Mailing services for the past three to five years, including a Client Reference Listing
List current projects under contract, contract period and duration, and number of staff assigned
Insert proposed cost schedule as seen in the 'Schedule of Bid Items' section
Complete the 'Vendor Questionnaire' section
Provide name, title, phone, and email of authorized representative
State number of years as a Contractor in this type of work
List names and titles of all officers, partners, or individuals doing business under trade name
Indicate business type (Sole Proprietorship, Partnership, Corporation) and submit evidence of compliance for fictitious names
Upload certificate for Partnership or Corporation if applicable
Upload document for Foreign (Non-Florida) Corporation if applicable
Describe the last project of this nature completed, including project value
Disclose if ever failed to complete work awarded
Confirm personal inspection of the proposed WORK and a complete plan for its performance
List CM's or GC's worked for within the past three years, including contact person's name, phone, and email
List three significant projects completed within the past five years, including project name, location, date completed, contract amount, contracting Agency/Owner, and contact person's name, phone, and email
List pertinent experience of key individuals of the organization
State name and licensing of individual with personal supervision of the Work
List equipment owned and available for the Work
List equipment to be purchased for the proposed Work
List equipment to be rented for the proposed Work
Attach certificate of status, competency, and/or state registration
Confirm acknowledgment that information in response is true and will be relied upon by City
Disclose if any part of the Work will be sublet
