Upload a complete electronic copy of the bid to the Triple R bidder portal by March 20, 2026, 2:00 PM
Provide a 5% Bid Bond if the total base bid price is $31,321.00 or more
Include the full legal name, business address, telephone number, and legal entity type of the Bidder in the bid
Ensure each copy of the bid is signed by a legally authorized person
Complete and submit the Bid Signature Page (Appendix B)
Complete and submit a Bill of Materials (Appendix C) for each RFP section bid on
Include a properly completed, signed, and notarized Familial Disclosure Affidavit (Appendix D)
Include a properly completed, signed, and notarized Iran Linked Business Affidavit (Appendix E)
Provide a list of at least four references, including one school or school district, for similar work performed within the past three years
Provide a written explanation for any variance from the specifications in section 8.0.0
Comply with all applicable federal, state, and local codes and regulations
Exclude sales tax from the bid
Demonstrate sufficient resources to complete the project within the allotted timeframe
Include the USF Service Provider Identification Number (SPIN) in the bid
Identify USF eligible products and/or services separately from ineligible ones
Itemize all materials, components, and equipment individually, including manufacturer, part number, unit cost, and quantity
Adhere to the Lowest Corresponding Price (LCP) requirements
Furnish all labor, supervision, tooling, and miscellaneous mounting hardware and consumables for the Network Electronics system
Test all hardware for defects in installation and to verify performance
Provide installation, administration, testing, and "As-Built" documentation
Provide a written guarantee/warranty for three years covering workmanship, components, performance, and follow-on support after project completion
Provide a project implementation schedule that adheres to the timeline requirements
Execute the awarded contract within ten days of being notified of acceptance
Furnish a Performance Bond and a labor and material payment Bond if total labor charges for the project equal or exceed $2,500.00
Comply with the State of Michigan policy regarding "Non-Discrimination of Employment" and the Prevailing Wage for State Projects Act (where applicable)
Provide all employees with picture ID and ensure they sign in and out at the main office for each school building location
Supply weekly work schedules by email no later than the Wednesday of the preceding week
Ensure all employees and subcontractors with direct and unsupervised access to students have undergone and passed a criminal history record check
Provide written certification that all on-site personnel have successfully passed background screening
Be prepared to provide supplemental information (supplier names, performance records, equipment list, financial statements, project descriptions, litigation information) within ten days if requested
Have materials on site and available for inspection prior to the start of the projects
Ensure all equipment used for the project complies with the specified part numbers
Invoice 100% upon completion of the SOW and acceptance by the District, with payments made NET 30 days after acceptance of "AS-BUILT" diagrams and cable certification testing results
Submit "AS-BUILT" diagrams in both paper and electronic copy (preferably Microsoft Visio)
Submit proof of performance by submitting cable certification testing results
Submit an itemized invoice based upon the installation schedule and supporting documentation
Furnish required guarantees and waivers of lien before final payment
Furnish a written guarantee of services and/or products for at least three years after final payment
Furnish written warranty documentation for cable, connectivity, and network electronics for three years
Correct any work that fails to conform to contract requirements due to faulty materials, equipment, or workmanship within three years of completion
Submit a Certificate of Insurance with DSISD Consortium named as an additional insured on the policy
Provide Workers Compensation and Employer's Liability Insurance
Provide Automobile Liability Insurance with a limit of $1,000,000 per accident
Provide Commercial General Liability Insurance with specific limits ($1,000,000 Occurrence, $2,000,000 General Aggregate, $2,000,000 Product-Completed Operations Aggregate)
Provide evidence of insurance coverage on Accord Certificate form 25 or 25-S, indicating a "Best" rating of B+ VII or better for insurance carriers
Provide an Installation Floater for the value of the contract
Include technical services and a 3-year warranty in all bid prices for eligible equipment
Bid for Basic Maintenance of Internal Connections (BMIC) separately, identifying costs, estimated hours, and materials for the period July 1, 2026, through June 30, 2027
