Printing services are needed for motor vehicle forms with specific dimension and perforation requirements for a government authority in Idaho.

Maintain Commercial Automobile Liability and Commercial Umbrella Liability insurance with a limit of not less than $1,000,000 each accident
Maintain workers compensation and employer's liability with limits not less than $1,000,000 each accident for bodily injury, $1,000,000 disease policy limit, and $1,000,000 disease, each employee
Provide a certificate of workers compensation insurance issued by a surety licensed in Idaho or an extraterritorial certificate approved by the Idaho Industrial Commission
Include the State of Idaho, ITD, and its divisions, officers, and employees as additional insured on liability insurance coverage
Provide proof of additional insured status via Certificate of Insurance (COI)
Ensure written notice is delivered for cancellation, material change, potential exhaustion of aggregate limits, or intent not to renew insurance coverage(s)
Obtain insurance from insurers rated A-VII or better in the latest Bests Rating Guide and authorized to transact business in Idaho
Be financially responsible for all deductibles, self-insured retentions, and/or self-insurance
All policies must contain waivers of subrogation
For claims made policies, the Retroactive Date must be before the contract date, and insurance must be maintained for at least three (3) years after completion of work, or purchase 'extended reporting' (tail) coverage for a minimum of three (3) years
Submit bids electronically via IPRO
Submit the Bid Schedule (Attachment 2) and the State of Idaho Signature Page
If the bid contains trade secret information, submit a redacted copy and a separate document listing the redacted information
Prices must be firm for one (1) year from the effective date of the Contract
No subcontractors are allowed without prior written permission from the Idaho Transportation Department
Maintain all records necessary to properly account for payments for at least three (3) years after contract termination
Allow State and Federal auditors and State purchasing staff access to all records relating to the Contract
Comply with Idaho Code §s 60-101 and 60-103 regarding printing preference (printing within Idaho unless cost is 10% higher than outside the State)
Comply with Idaho Code §67-2349 regarding Idaho Reciprocal Preference Law (10% additional cost for evaluation for out-of-state printing)
Deliver forms FOB Destination, freight prepaid and allowed, to the specified location within thirty (30) days after receipt of order (ARO) or as coordinated
Meet minimum order quantities for each form (192 cases for ITD Form 3582, 48 cases for ITD Form 3584, 192 cases for ITD Form 0502)
Contractor must be registered with the Idaho Secretary of State if required to conduct business in Idaho
If payments exceed $100,000 and the contractor employs ten (10) or more persons, certify that it is not currently engaged in, and will not engage in, a boycott of goods or services from Israel
Certify that it is not currently owned or operated by the government of China and will not be for the duration of the Contract
If payments exceed $100,000 and the contractor employs ten (10) or more persons, certify that it is not currently engaged in, and will not engage in, a boycott of various industries (fossil fuel-based energy, timber, minerals, hydroelectric power, nuclear energy, agriculture, or firearms)
