Commercial General Liability endorsements: Annual Aggregate 'Per Project/Job', Waiver of Subrogation in favor of the City, Primary and non-contributory basis, City of Dania Beach as Additional Insured
Business Automobile Liability limits: Combined Single Limit $1,000,000 (Any Auto/Owned Autos, Hired and Non-Owned Autos, Any One Accident)
Workers' Compensation Limits: Statutory - State of Florida, Waiver of Subrogation in favor of the City
Employers Liability Limits: $100,000 bodily injury per accident, $100,000 bodily injury per employee, $500,000 bodily injury policy limit
Umbrella/Excess Liability (Excess Follow Form) to meet required limits
Subcontractors must comply with all insurance requirements
Insurance policies must provide 10 days' written notice of cancellation or material change to the City
Insurance carrier must be rated no less than 'A-' (management) and 'Class VII' (financial strength) by Best Insurance Guide
Contractor must comply with Florida Department of Commerce industrial safety rules and OSHA standards
All materials and equipment supplied must meet federal and state requirements, including OSHA
Contractor must have FDOT Approved Intermediate Maintenance of Traffic Certification or higher
Contractor must warrant all materials and workmanship for a minimum of one year
Contractor must ensure all irrigation operations comply with state and local water management district regulations and employ best management practices
Contractor must comply with the Florida Trench Safety Act and all applicable OSHA regulations for trenching and excavation work
Bid must include a Letter of Transmittal
Bid must include an anticipated timeline to begin delivery of services
Bid must include a breakdown of the costs for the delivery of services
Bid must include resumes of key personnel assigned to the project and their role descriptions
Bid must include a list of five similar projects performed in South Florida with specific details
Contractor must be able to provide increased levels of service (additional hours) at the same cost per hour
