DESCRIPTION

A government authority in Rochester, New York, is seeking vendors to provide comprehensive information, referral, and prevention services aimed at improving the community's awareness of disability-related issues and available resources. Services will address the needs of adults and school-age youth by working with individuals, students, and families at risk or experiencing early signs of potential mental illness. The goal is to prevent the escalation of symptoms through prevention activities that may occur in community or school-based settings.

The selected vendor will be responsible for enhancing outreach efforts and improving access to treatment and prevention services for older adults with mental illness. Additional responsibilities include providing information and advocacy for individuals with intellectual or developmental disabilities (I/DD) and their families, as well as offering public education on disability awareness to organizations wishing to foster inclusion for people with disabilities. The RFP also seeks providers capable of school-based services that identify children beginning to display school adjustment difficulties, pairing them with in-school supports to promote both individual and group skill development.
