Applicant must understand and accept RFA requirements
Applicant must agree to be bound by Contract terms in Attachment G
Applicant must acknowledge receipt of any and all Addenda
Application must be a Firm Offer for 180 days
If awarded, Applicant must agree to perform the scope of work and meet performance standards
Applicant must not be in violation of any state tax laws
Applicant must not discriminate in its employment practices
Applicant must not discriminate against certified businesses when awarding subcontracts
Applicant must have or will have a written policy preventing sexual harassment, sexual assault, and discrimination against protected classes
Applicant must comply with ORS 652.220 regarding wage discrimination
Contractor may not prohibit employees from discussing compensation
Applicant must be in compliance with Oregon Laws 2018, Chapter 88 (HB 4155) regarding broadband internet access service
Applicant and its employees, agents, and subcontractors must not be included on the 'Specially Designated Nationals and Blocked Persons' list
Applicant and its employees, agents, and subcontractors must not be included on government-wide exclusions lists in the System for Award Management (SAM)
Applicant must certify no actual or potential conflict of interest exists
All application contents must be truthful and accurate, and prepared independently
Applicant must acknowledge that these certifications are in addition to any required in the Contract and Statement of Work
Applicant must have appropriate financial, material, equipment, facility, and personnel resources or ability to obtain them
Applicant must have completed similar contracts within time and budget, without claims, within the last five years
Applicant (including partners, shareholders, or major subcontractors) must not have been criminally or civilly charged, indicted, or convicted in connection with public contracts, antitrust, embezzlement, theft, forgery, bribery, falsification of records, false statements, tax evasion, or receiving stolen property within the last three years
Applicant must not have had any contracts terminated for default by a government agency within the last three years
Applicant must not have had any lawsuits filed against it by creditors or involving contract disputes within the last three years
Applicant must not have any outstanding or pending judgments against it
Applicant must not be experiencing financial distress or difficulty securing financing
Applicant must have sufficient cash flow to fund day-to-day operations throughout the proposed contract period
Applicant must not have filed a bankruptcy action, filed for reorganization, made a general assignment of assets for the benefit of creditors, or had an action for insolvency instituted against it within the last three years
Applicant must have all required licenses, insurance, and/or registrations and be legally authorized to do business in the State of Oregon
Applicant must possess an unexpired Pay Equity Certificate if employing 50 or more full-time workers and the prospective contract price is estimated to exceed $500,000
Applicant must submit Attachment F 'Disclosure Exemption Affidavit' if wishing to claim exemption from public disclosure
If Attachment F is submitted, an identical but redacted version of the Application must also be submitted
Applicant must identify exempt portions on Exhibit A to Attachment F
Attachment F must be complete, properly completed, and legally notarized
Contractor must be in compliance with all insurance requirements in Exhibit C of the Contract
Contractor must deliver the required Certificate(s) of Insurance within 30 days of Contract execution
Contractor must acknowledge the Oregon False Claims Act applies to any claim made under the Contract
Contractor must certify that no false claims will be made
Contractor must have complied with the tax laws of the State of Oregon and applicable political subdivisions
Contractor must comply with all tax laws of Oregon and applicable political subdivisions throughout the Contract duration
Contractor must ensure information in Section 5.a. of the Contract is true, accurate, and correct
Contractor must not have discriminated and will not discriminate against minority, women, or emerging small business enterprises in obtaining subcontracts

Contractor is responsible for all federal and state taxes applicable to compensation
Contractor must perform all Work as an Independent Contractor
Contractor must have the power and authority to enter into and perform the Contract
Contractor's obligations under the Contract must be valid and binding
Contractor must possess the skill and knowledge to perform the Work professionally
Contractor must be qualified, professionally competent, and duly licensed to perform the Work
Contractor must have prepared its proposal independently
Contractor must not be compensated for Work by any other state or federal agency
Contractor must comply with all federal laws, regulations, and executive orders applicable to the Contract
Contractor must comply with Clean Air, Clean Water, and EPA Regulations if the contract exceeds $100,000
Contractor must comply with applicable mandatory standards and policies relating to energy efficiency
Contractor must certify that no federal appropriated funds have been paid for influencing or attempting to influence federal officials regarding the contract
Contractor must complete and submit Standard Form LLL 'Disclosure Form to Report Lobbying' if other funds are used for lobbying
Contractor must include certification language in all subawards
Contractor must not use federal funds for publicity or propaganda purposes
Contractor must not use federal funds to pay salary or expenses for lobbying activities
Contractor must not use federal funds for advocacy or promotion of gun control
Contractor must not use federal funds for any activity that promotes the legalization of certain drugs or substances
Contractor must comply with the Resource Conservation and Recovery Act
Contractor must comply with applicable audit requirements and responsibilities
Contractor must have a single organization-wide audit if expending $750,000 or more in federal funds in a fiscal year
Contractor must not permit any person or entity listed on the non-procurement portion of the General Service Administration's 'List of Parties Excluded from Federal Procurement or Non-procurement Programs' to be a subcontractor
Contractor must comply with the Pro-Children Act of 1994

Applicant must describe how its Supervisors and Direct Care Staff meet or exceed requirements in subsection 3.3 'Staffing Requirements and Responsibilities'
Applicant must include a copy of its active Child Caring Agency license
Applicant must include complete position descriptions for open or unfilled Key Person positions
Applicant must describe its business management experience and strategies for managing a CCA or similar setting
Applicant must describe its experience and strategies for ensuring fiscal responsibility and solvency
Applicant must identify staff or board members with fiscal responsibilities
If a new business (established within 24 months), Applicant must include a copy of a business plan
Applicant must provide a comprehensive implementation and management plan for the IRC Program
Applicant must describe its philosophy of positive youth development and integration of trauma-informed practices
Applicant must describe its ability to provide skilled and individualized services for intake, assessment, service, and case/transition planning
Applicant must describe its plan to adequately staff for the service
Applicant must describe the facility or building for IRC Services, ensuring a trauma-informed environment and client safety
Applicant must describe its approach to meeting reporting and documentation requirements, including budget management and data tracking
Applicant must create a 1-year line-item budget for IRC Services

The Individual Residential Care Services (IRC) Program must operate within a licensed setting or 24-hour residential facility, providing care to one client at a time. Services will continue while working to secure access to recommended care or until a permanent care plan is established.
