The government authority in San Diego, California is seeking a vendor to provide mobile in-home outreach and engagement services targeting individuals with serious mental illness (SMI) who are hesitant to seek outpatient mental health care, as well as their family members and caretakers. The selected agency teams will deliver services including in-home assessment, crisis intervention, short-term case management, peer and family or caretaker support, psychoeducation, and linkage to outpatient mental health, rehabilitation, recovery, and other support services as needed.
The program emphasizes support for Medi-Cal Transformation, Program Integrity, and Quality Improvement requirements. Documentation provisions must be met as part of service delivery. Pre-proposal meetings and opportunities for questions are scheduled prior to submission deadlines. The intent is to increase engagement and support for individuals with SMI and help reduce barriers to accessing essential services.
