Seeking a qualified vendor to provide software license renewals, maintenance, and technical support services for IBM Maximo over a three-year term in California.

Describe how competitive pricing for IBM Maximo® Software Licensing will be ensured
Describe management approach for Task Order management, communications, quality, costs, and schedule
Describe approach and experience for optional IBM Maximo® integration with Oracle Database/ERP
Describe approach and experience for optional IBM Maximo® update to current version
Describe prior experience on similar public projects
Provide five (5) project descriptions from the past 5 years demonstrating similar work, including specific roles, contract amount, term, and client reference
Identify Key Personnel and their roles
Provide resumes for Key Personnel, including years of experience, education, licenses/certifications, and relevant project experience
Confirm acceptance of RFP terms and conditions
Confirm acceptance of SMART's Sample Agreement terms and conditions
Confirm review of insurance requirements and agreement to obtain required insurance
Confirm no known or potential organizational conflicts of interest, or provide a report if conflicts exist
Submit W9 Tax Form or Equivalent
