A qualified vendor is sought to deliver outreach and education services for hospitals and nursing homes throughout New Hampshire. The scope of work includes participating in pilot initiatives, rapid-cycle testing, and evaluation activities as directed by the Department, with the aim of identifying models that can be replicated for statewide implementation.
The selected organization will be responsible for maintaining structured coordination with Aging and Disability Resource Centers (ADRCs) through monthly case review meetings, shared protocols, and regular cross-training. Additional responsibilities include utilizing person-centered approaches to improve transition outcomes and gathering data in support of the State’s Division of Long-Term Supports and Services (DLTSS) post-demonstration sustainability plan. Comprehensive outreach and educational activities for all hospitals and nursing homes within the designated region are required to promote improved care and service delivery.
