The State of New Hampshire is seeking a vendor to provide outreach and education services targeting hospitals and nursing homes. This initiative aims to enhance existing systems and support Medicaid-related objectives, with a focus on improving outcomes for children, families, individuals, and seniors in areas such as mental health, developmental disabilities, substance abuse, and public health.
A central goal of these services is to promote the use of home and community-based services (HCBS) over institutional long-term supports and services (LTSS). The selected vendor will work to eliminate barriers preventing the flexible use of Medicaid funds, supporting Medicaid-eligible individuals in receiving care in their preferred settings and ensuring the continued provision of HCBS.
Key responsibilities include building and maintaining relationships with hospital and nursing home discharge planners and social workers, raising awareness about available support services, and proactively identifying individuals at increased risk of institutionalization. The vendor will also implement early intervention and diversion strategies, which may involve coordinating secure record transfers, managing the transition of data and services, and working collaboratively with third-party consultants to ensure effective service transitions.
