Proposals must be received no later than 4:00 PM on April 27th, 2026.

Attendance at a Falmouth Human Services Committee Meeting no later than April 6th, 2027.

DESCRIPTION
The selected vendor will provide homeless prevention and outreach services, focusing on housing stabilization case management for residents who are at risk of homelessness. Services are to include personalized case management support designed to help individuals maintain stable housing and avoid entering homelessness.
Additionally, the vendor will conduct outreach and provide case management for residents who are currently homeless. This includes active engagement with the homeless population, assessing their needs, offering support, and facilitating access to community resources and shelter solutions.
The primary objective is to reduce homelessness by stabilizing housing situations for at-risk individuals and providing necessary outreach to those currently without stable housing.
