DESCRIPTION


The selected provider will deliver homeless outreach, navigation, and rehousing services aimed at engaging and supporting individuals experiencing unsheltered homelessness within designated service areas. Responsibilities include connecting clients to supportive services, emergency shelter, interim and permanent housing, and building strong relationships with local community organizations, residents, and businesses to cultivate a collaborative support network for unhoused individuals.

Service components encompass assisting unsheltered individuals to obtain necessary documentation for accessing shelter and housing, facilitating their transition into permanent housing, and leveraging additional community rehousing resources. The program seeks to engage and serve people in non-habitable environments and will provide housing-focused services, including screening, intake and assessment, housing problem solving, case management, stabilization assistance, and navigation to financial and mainstream resources.

Providers will offer voluntary case management to support client stabilization, health, and income through connections to mainstream and community-based services, public benefits, ongoing recovery and health resources, and other longer-term support mechanisms. The contract will span one year, with opportunities for interested organizations to attend a non-mandatory pre-bid meeting and submit questions prior to the bid deadline.
