DESCRIPTION
The government authority in Philadelphia, Pennsylvania seeks a vendor to provide home delivered meal (HDM) services for older adults who are homebound and unable to procure or prepare their own meals. These services include delivering authorized meals and related nutrition support directly to the homes of eligible seniors.
Eligibility requires that the older adult is homebound and does not reside with or have access to informal support systems capable of meeting their daily meal and grocery shopping needs. The contract period for these services will be three years.
