DESCRIPTION


The government authority in Memphis, Tennessee seeks vendors to provide home and community–based services. Applicants must submit comprehensive documentation including organizational history, governance details, organizational chart, W–9 form, details of organizational capacity, mission and values, personnel structure, qualifications, training curriculum, audited financial statements, proof of business status, valid certificate of insurance, verification of workers compensation insurance, bank references, credit references, organizational conduct policies, and internal monitoring tools.

Vendors must demonstrate a record of accurate and timely billing, provide timelines for service implementation, establish policies for conducting and maintaining background checks, and submit schedules of holidays observed. Accurate reporting is required, and organizations must verify that they have measured Customer Satisfaction and obtained documentation proving at least an 80% satisfaction rate among consumers. Organizations must attest their ability to provide services across all proposed counties and ensure that costs remain below the maximum allowable amount.

All questions regarding the RFP must be submitted by March 20, 2026. The initial contract will be for two years.
