DESCRIPTION

The government authority in New Hampshire is seeking a vendor to deliver grant evaluation services for its rural health transformation program. The selected firm will be responsible for formulating a comprehensive five-year evaluation plan that aligns with agency priorities, applicable initiative goals, CMS expectations, and reporting requirements, with annual updates as needed.

Core tasks include performance monitoring through regular data collection, metric analysis, and monthly review of initiative reports from agency hubs.
