RFP for vendors to supply fundraising items and related services on an as-needed basis in Texas.

Submit proposals through the District's online bidding portal or hard copy to the purchasing department
Proposals must meet or exceed minimum specifications
Late responses will not be considered
Submit samples or catalogs for alternate brands
Quote FOB Destination (if applicable)
Provide full name and address of firm
Manually sign response and show title/authority to bind the firm
Responses cannot be altered or amended after opening time without approval
All prices must exclude taxes
State delivery time; default is 14 calendar days
All items must be new, in first class condition, and manufacturer's latest model
No substitutions or cancellations without written approval
Acknowledge and agree to contract terms
Acknowledge and agree to proposal submission instructions
Acknowledge and agree to standard terms and conditions
Certify compliance with EDGAR Certifications (2 CFR Section 200 Required Provisions)
Certify agreement to enter into contract and provide services per specifications
Certify accuracy of certifications, including Felony Conviction Notice
Affirm compliance with Equal Employment Opportunity Commission and American Disability Act standards
Certify proposal submitted independently without collusion
Provide ordering and/or remit to address(es) if different from W9 address
Attach completed W9 form
Provide five references
Indicate if a Minority/Women Owned Business
Provide categories/commodities based on NIGP Codes
Select any purchasing cooperatives awarded a contract with
Certify felony conviction status (Publicly-held, not owned/operated by felon, or owned/operated by felon)
If owned/operated by felon, provide name(s) and details of conviction(s)
Certify felony conviction information
Provide Certificate of Residency information (Resident Bidder or Nonresident Bidder)
If nonresident bidder, state principal place of business, whether resident state requires Texas bidders to underbid, and amount/percentage
Acknowledge and certify non-collusion statement
Certify criminal history record information regarding continuing duties at GCCISD
Certify criminal history record information regarding direct contact with students
If no continuing duties or direct contact, agree to be considered visitors on school campuses

Provide a brief description of how the fundraiser works
Provide profit percentage(s) for fundraising activities
Confirm if upfront purchases are required and if a benefit check is provided
