Comply with Kentucky statutes (KRS 45A, 160.380, 45A.395, 45A.490-494)
Comply with Federal regulations (Clean Air Act, Federal Water Pollution Control Act, FERPA, Davis-Bacon Act, Contract Work Hours and Safety Standards Act, Debarment and Suspension, Byrd Anti-Lobbying Amendment, Buy American provisions)
Submit proposals by the due date and time
Proposals must be signed by an authorized representative
Proposals must be effective for sixty (60) days from closing date
No conflicts of interest, gratuities, or kickbacks to employees of the Board
Monitor the district website for any addendums or updates to the solicitation
Companies with nexus in Lexington, KY, must be current on all filings and payments of Occupational License and Net Profits Tax for Schools
No discrimination on the basis of race, color, national origin, sex, genetic information, disability, religion, age, political affiliation, sexual orientation or gender identity
Prohibit consumption of alcohol or drugs, use of tobacco products, or possession of firearms while on a job for FCPS
Not be listed on government-wide exclusions in the System for Award Management (SAM)
Contractors providing services to students on a regularly scheduled and continuing basis must submit to a state criminal history background check and provide a letter from the Cabinet for Health and Family Services regarding child abuse/neglect findings
Furnish Workers' Compensation and Employer's Liability insurance
Furnish Public Liability insurance ($1,000,000.00 minimum)
Furnish Property Damage insurance ($1,000,000.00 minimum)
Include a Certificate of Insurance with the proposal
Insurance company must be authorized to transact business in the Commonwealth of Kentucky
Insurance company must have a general policyholder's rating of 'A-' to 'A+' and a financial size category of at least 'Class XIII'
Provide secondary hired or non-owned liability coverage
Assume responsibility for securing other needed information when the Board's representative has turned in a complete Accident or Proof of Loss Report
Timely furnish Quarterly, Biannual, Annual, and Year-to-date Loss Runs
Submit the proposal as a single PDF file
Include signed RFP documents (Pages 1 and 2) and the checklist from page 14
Include three (3) references from similar sized agencies or school districts
Include a Technical Proposal covering the scope of work and specifications
Include a Cost Proposal with overall total price and unit pricing breakdown
Include any information considered as Added Value
Include a completed Cover page
Include Taxpayer Identification Number (if not a Corporation)
Include a Resident Bidder Affidavit if declaring Resident Bidder Status
Include a completed W9 form
For hard copy submissions, include a USB flash drive with a digital copy of the proposal
