Provide uniquely numbered delivery tickets signed by Department's representative, containing purchase order number, sub-purchase order number, vendor code, City of Los Angeles Business Tax Registration Certificate number, material identification, quantities, unit prices, extensions, discount, federal excise tax discount (if applicable), sales or use taxes, and total price.
Render an invoice accompanied by one copy of each delivery ticket signed by Department's representative for all deliveries made during the month. Invoice must include delivery ticket number, date, total price, purchase order number, sub-purchase order number, vendor code, City of Los Angeles Business Tax Registration Certificate number, and material identification. Sales or use taxes must be separately stated.
Include a signed attestation with every invoice.
Prices quoted must include applicable duties.
Prices quoted must be exclusive of federal excise taxes.
Prices quoted must be FOB Destination.
Prices shall remain firm for 90 calendar days from the bid closing date.
Submit a sample for testing within 5 calendar days if requested.
Submit annual recycled-content report form (July 1 to June 30) by July 15.
If claiming Local Business Preference Program (LBPP), provide all required information in the 'Questionnaire for Equipment, Goods, or Materials Form' before bid closing.
State if cooperative purchasing with other governmental agencies is allowed.
State that conflicting terms and conditions from the price list will not apply to this bid and LADWP.
