Provide proof of insurance, including Commercial General Liability, Auto, and Contractor's Pollution Liability, with the Purchaser named as insured.
Provide proof of health and safety certifications for key project personnel in Worker Health and Safety Awareness, WHMIS, and Hazardous Materials Awareness (Asbestos).
Attend a mandatory site meeting.
Submit proposals online via Euna and in English only.
Comply with Anti-Racist, Anti-Discrimination, and Anti-Oppression Commitment.
Highlight Environmental Social and Governance (ESG) programs, products, and/or services.
Direct all communications to the Bid Administrator via Euna's Q & A Board.
Execute the agreement within 10 business days if selected as the Preferred Proponent.
All prices must be in Canadian funds, exclusive of taxes, and remain firm for the intended term.
Provide detailed qualification statements for team members, including project manager and site superintendent, with relevant experience in healthcare facilities and similar renovation projects.
Submit a narrative/summary of the company's Health and Safety Policy/Program, addressing project-specific safety policies, corporate H&S policy statement, H&S training, access control, emergency preparedness/planning, and mitigating workplace injuries.
Provide at least three client references comparable in complexity and nature.
Submit a comprehensive methodology and approach for project completion, including plans for on-time/on-budget/accident-free delivery, health and safety in mental health/emergency departments, a detailed GANTT schedule, and risk mitigation strategies.
