Proposers must be registered on the City's eBid System to view and respond to solicitation documents.
Proposals must bear the electronic signature of an authorized officer.
Proposals must be submitted electronically via the eBid System by the deadline.
Insurance carriers must currently provide group insurance to at least two other Municipal entities with a minimum of 500 employees.
Proposers are encouraged to match current medical plan designs as closely as possible.
Proposals should include a minimum of $100,000 in annual Wellness Funds.
Proposers are encouraged to include technology funds in their proposal.
Proposers are encouraged to include performance guarantees, implementation guarantees, service guarantees, and network discount guarantees.
Proposals for ASO (Administrative Services Only) are highly recommended to provide multiple-year rate guarantees.
The awarded proposer must provide representatives to assist with implementation, open enrollment, employee communications, and ongoing routine plan administration.
Proposers must have the technological capacity to transmit and accept a HIPAA 834 5010 eligibility file with proper confirmation of receipt and discrepancy reporting.
Proposers must provide a detailed comparison of their proposed benefit plans to the City's current plan designs.
Proposers must describe their referral procedures.
Proposers must submit a comprehensive provider network analysis, including an interruption report and access analysis.
Proposers must describe their claims administration process.
Proposers must provide responses to all items listed in the City's Questionnaire and Fact Sheet.
Proposers must include their carrier standard contract in Word Document format.
Proposers must disclose any litigation within the past five years or include a letter stating no litigation exists.
Proposers applying for Tier 1 or Tier 2 Local Business status or listing local subcontractors must provide a valid City of Pompano Beach Business Tax Receipt.
Proposers must agree to the terms and conditions of the solicitation.
Proposers must acknowledge reviewing all addenda.
Proposers must certify compliance with drug-free workplace requirements (Section 287.087, Florida Statutes).
Proposers must disclose if any City employee is an owner, corporate officer, or employee of their business (Conflict of Interest).
Proposers must disclose if their company is a Local Business located within the City of Pompano Beach City Limits.
Proposers must certify that the company is not on the Scrutinized Companies that Boycott Israel List and is not participating in a boycott of Israel.
