Vendors are invited to provide evidence-based disease prevention and health promotion services for older adults residing in New York. The scope includes delivering programs and activities that educate and empower older adults to adopt healthy lifestyles, encourage positive health behaviors, and improve their functional status. These services aim to prevent or reduce the incidence of chronic diseases and falls among the senior population.
The initiative requires targeted outreach to vulnerable groups, including low-income and low-income minority individuals (such as Native Americans), older adults living in rural areas, those with the greatest economic need, individuals with limited English proficiency, seniors who are frail or have severe disabilities, those living with Alzheimer’s disease or related neurological disorders, as well as the caregivers supporting them. Programs should be rooted in scientific evidence, demonstrating effectiveness in improving health outcomes for older adults. Services may be delivered at various community locations throughout the region.
Vendors are encouraged to ensure that the services address the critical needs of these targeted populations.
