Dental Coverage Plan Insurance Services

The city of Sandy, Utah is requesting proposals for dental coverage plan insurance services for its participants.

Firm must be properly licensed to conduct business in the State of Utah and hold all necessary certificates.
A local service representative is required.
Firm must secure and maintain insurance as stated in the CSD Standard Terms and Conditions.
Professional Liability and Errors and Omissions insurance coverage of at least $1,000,000 per occurrence and in an annual aggregate, with CSD named as an additional named insured.
Commercial general liability insurance in the minimum amount of $1,000,000 per occurrence with a $2,000,000 general aggregate, with CSD named as an additional insured.
Commercial Blanket bond with a dishonesty sub-limit in the minimum amount of $100,000, with CSD named as an additional insured.
Proposers must attest to CSD HIPAA actions and compliance in writing.
Firm must have performed the same scope as outlined with large employers and in similar business environments.
All solicitation responses must be submitted electronically through Euna Procurement.
Intent to Bid and Confidentiality/Non-disclosure Agreements must be received by March 18th, 2026 5:00pm.
Proposals must be signed by the Firm's authorized official binding the RFP and the Firm for a period of ninety (90) days.
Each Firm shall submit only one (1) standalone proposal for this benefit that matches the current benefits.
Firms may submit a bundled proposal with the RFP #26-00069MM-P5 Vision Coverage Plan.
Firms may submit one (1) proposal that reflects unique plans or offerings.
Proposing Firms are expected to complete Schedules A-D and upload as part of their proposal, and Schedule F (price) should be uploaded as a separate file.
Price proposals should provide a multi-year rate guarantee with at least a two (2) year rate guarantee.
Local presence is preferred and will be taken into consideration.
Supplier must read and acknowledge Canyons School District Professional Services Terms and Conditions.
Contractor must certify registration and use of employment 'Status Verification System' (E-verify) if performing physical services within the State of Utah.
Contractors/Suppliers with 10 or more employees applying for $100,000 or more must submit a written certification regarding public contract boycott restrictions.
