The government authority in Minnesota seeks vendors to improve the overall quality of services delivered to people receiving customized living services. Providers must demonstrate capability to enhance service quality for persons participating in brain injury or community access for disability inclusion waivers, with services offered at multiple locations, each hosting six or more residents.
Responders must serve a high percentage of waiver participants, with eligibility for quality improvement grants requiring at least seventy-five percent of clients served to be waiver participants. Applicants should also be providers currently offering customized living services within the specified waiver programs. All questions regarding this RFP are due by March 23, 2026.
