Construction and Maintenance Services for Traffic and Utility Systems

Provide statewide (Illinois) and nationwide OSHA citations in the last 5 years (no willful or serious violations)
Provide current nationwide Incident, Frequency and Severity numbers
Provide detailed Emergency Response Capability Plan, including nationwide staffing data
Submit separate rate sheets for each type of work provided
Register as a vendor on the City of Springfield website
Submit bids electronically via the City's Bid Portal in PDF format
Comply with Illinois Underground Utility Facilities Damage Prevention Act and JULIE requirements
Maintain personnel and equipment availability on a twenty-four (24) hour, seven (7) day per week basis
Maintain an active dispatch contact reachable at all times
Ensure sufficient staffing levels to meet simultaneous work requests
Acknowledge emergency calls within 15 minutes, priority calls within 30 minutes, and routine work within one (1) business day
Ensure crew is en route for emergency work within 30 minutes and priority work within two (2) hours
Demonstrate ability to increase crew count within twelve (12) hours during storms or major system events
All equipment must be fully operational, properly licensed, inspected, and compliant with OSHA, ANSI, DOT, and applicable state requirements
Comply with all applicable provisions of Federal, State, and Municipal safety laws
Provide adequate flagmen, signs, barricades, and other protective devices when working along public thoroughfares
Restore any and all damaged property at own expense
Obtain own headquarters for men and equipment in reasonable proximity to job sites
Remove all excess earth, rock, debris, underbrush, and other useless material from the site
Maintain adequate protection of work and Owner’s property from damages
Leave the work site in a safe manner at the end of each day’s work
Limit movement of crews and equipment to cause as little damage as possible to property
Perform final clean-up to restore all areas and work sites to former condition
Do not dispose of concrete, soil, contaminated soil, or soil millings on Owner’s property
Return all scrap wire, excess, or faulty materials to the Owner
Maintain a competent foreman or superintendent at the Site, subject to prior approval by the Owner
Personnel must have at least two years of experience in the applicable area of the bid (apprentices under direct supervision of 4+ years experience)
Personnel must present a proper appearance and conduct professionally
Provide proper tools and equipment to do the work
All Contractor personnel must conduct themselves in a courteous, professional, and customer-first manner
All employees of the Contractor must have a valid driver’s license
Personnel must wear appropriate uniforms or attire identifying the Contractor
Personnel must carry and display a photo identification badge
Personnel must clearly identify themselves as working on behalf of CWLP
Regular working hours shall be from 7 am to 11 am and 11:30 am to 3:30 pm, Monday through Friday (overtime requires Project Manager approval)
Obtain permission from owners before accessing private property and written consent for alterations
Work on lines of 15KV or less will be done energized when lines cannot be taken out of service
Comply with all applicable laws concerning the bidder’s entitlement to conduct business in Illinois
Comply with all applicable provisions of the Prevailing Wage Act
Comply with Subchapter VI (“Equal Employment Opportunities”) of Chapter 21 of Title 42 of the United States Code and Federal Executive Order No. 11246
Have a valid Federal Employer Identification Number or Social Security Number
Have a valid certificate of insurance showing general liability, professional liability, product liability, workers’ compensation, completed operations, hazardous occupation, and automobile
Bidder and all subcontractors must participate in applicable apprenticeship and training programs approved by and registered with the United States Department of Labor’s Bureau of Apprenticeship and Training
Submit a signed affidavit stating that the bidder will maintain an Illinois office as the primary place of employment
Must not have any judgment or convictions of federal, state, or local law or regulation, and disclose any pending cases
Provide proof of conformance with any applicable Federal/State/Local permits, licenses, certifications
Furnish a satisfactory performance bond and a payment bond (up to 100% of the contract sum) if required
Insure all Owner-owned equipment used under this Contract against loss or damage on a replacement cost basis
Provide proof of insurance naming the Owner as additional insured and loss payee for Owner-owned equipment
Contractor is solely responsible for any deductibles applicable to claims involving Owner-owned equipment
Indemnify, defend, and hold harmless the Owner for claims arising out of or related to the Contractor’s use, operation, or possession of Owner-owned equipment
Guarantee work against any defects due to poor workmanship for one year after acceptance
Provide Material Safety Data Sheets for all hazardous materials furnished or associated with the Work
Provide written notice of hazardous materials to local fire, medical, and law enforcement agencies, and Owner
Provide labeling of hazardous materials and training of employees in safe usage
Immediately notify Owner if hazardous materials are discovered on the Work site
Comply with OSHA Fall Protection Standard 29 CFR 1926 Subpart M
Submit a fall protection plan prior to starting Work operations that require fall protection
Use full body harnesses, lanyards with shock absorbers and locking type snap hooks (at least two lanyards for 100% fall protection)
Comply with OSHA Standard 1926.502(b) for guardrail systems (excluding manila, plastic, or synthetic rope)
Honor Owner’s Designated Confined Spaces and meet OSHA requirements
Provide equipment to perform own air monitoring for confined spaces
Conform to Owner’s hot work permit requirements when burning, cutting, grinding, or welding
Conform to Owner’s equipment clearance order procedures
Conform to Owner’s asbestos abatement requirements
Treat all insulation and unidentified thermal insulation coverings as asbestos containing material
Notify Owner if any material suspected of containing asbestos is encountered
Permit, facilitate, and comply with all reasonable or lawful requests or inspections from the Owner, its agents, authorized insurance company inspectors, or governmental bodies
Comply with the Owner’s tagging procedures before working on equipment
Implement and maintain a written safety, health, and accident prevention program
Appoint a qualified and experienced Safety and Health Representative
Maintain accurate accident and injury reports and furnish monthly summary to Project Manager
Maintain accident rates below 25 percent of the national average for construction
Submit a written corrective action plan if accident rates exceed the Work safety goal
Hold regularly scheduled meetings to instruct personnel and subcontractors' personnel in safety and health practices
Furnish safety and health equipment and enforce its use
Assist Project Manager in administering a drug detection and prevention program
Employees under the influence or in possession of illegal drugs or alcohol will be immediately removed from the Work site
Employees subject to blood/urine test upon reasonable suspicion or involvement in an accident
Onsite calorimetric alcohol saliva testing permitted, with positive tests confirmed by EBT or blood test
Employees failing drug/alcohol test or refusing to cooperate will be banned from the Work site for 1 year (rehire possible after approved rehabilitation program)
Employees entering, while working, and leaving the Work site will be subject to search and inspection
All cranes in use inspected monthly by a competent person, with reports submitted to Project Manager
Submit current annual crane inspection reports by a third-party certified inspection service prior to placing crane in service
Submit a completed MBE/FBE Utilization Form (Attachment A) or an approved M/FBE Participation Waiver Request (Attachment B) with the bid
If claiming local bidder preference, certify by signature a physical presence with the City in accordance with City of Springfield Code of Ordinances, Section 38.45
Certify that products offered conform to the definition of 'Domestic Product' for preference
Certify that the bidder and any subcontractors do not employ unauthorized aliens
If a corporation, limited liability company, or partnership, provide a copy of current certification of good standing from the State of incorporation
If a foreign entity, register with the Illinois Secretary of State and provide proof of registration
Submit a complete, accurate, and truthful listing and description of all citations, complaints, summons, decisions, determinations, judgments, or other allegations or findings relating to any violation of State or Federal laws
Submit weekly certified payroll reports identifying actual residence of every employee
Comply with the minimum percentage of total work hours performed by eligible residents (50% for contracts with estimated value of $100,000 or more)
Submit a written equal employment opportunity commitment and a $50 administrative fee
Maintain nondiscriminatory employment practices
Make good faith efforts to meet MBE/FBE subcontracting and trade participation goals
Provide compliance evidence and workforce data upon request
Distribute commitment copies to recruitment personnel
Require subcontractors to submit similar commitments
Submit compliance reports and allow access to records for City investigation
