DESCRIPTION


The government authority located in Providence, Rhode Island is seeking organizations or collaborative groups to provide a Connections Mainstream Benefits Programming Service. This initiative focuses on administering evidence-based models such as SSI/SSDI Outreach, Access, and Recovery (SOAR), aimed at connecting individuals experiencing or at risk of homelessness to mainstream public benefits. Target populations include those at risk of homelessness and individuals with mental illness, medical impairments, and/or co-occurring substance use disorders.

Providers will be responsible for performing start-up activities, processing referrals, assessing eligibility, maintaining waitlists as needed, and assigning benefits specialist staff to guide eligible individuals through the benefits application process. Additionally, the responsibilities include conducting internal quality assurance reviews of applications, maintaining regular participant communication regarding application statuses and next steps, and ensuring benefits services complement rather than replace primary case management. The contract will have a one-year term.
