DESCRIPTION


The County is seeking information from organizations regarding the availability, capabilities, and implementation considerations of a comprehensive digital platform designed for client engagement and care coordination. This solution should support County residents and service providers working with individuals who have behavioral health needs, co-occurring conditions, and other serious chronic medical conditions, including substance use disorders.

The platform should encompass mobile and web applications that facilitate daily check-ins for medication adherence, mood and stress monitoring, appointment reminders, secure messaging, telehealth services, and resources for education and self-management. Additionally, the solution should offer web portals for care teams such as clinicians, case managers, and support staff, featuring tools for triage and alerting, task and workflow management, and population-level data dashboards.

Other key capabilities include risk stratification, automated flagging of risks, and human-in-the-loop decision support. Advanced analytics functions are also sought to assist in program evaluation and informed service planning.
