DESCRIPTION
This RFP seeks qualified vendors to provide community living supports, personal care, and respite services within licensed specialized residential settings in Grand Rapids, Michigan. Services are intended for adults with intellectual and developmental disabilities, as well as adults with severe mental illness.
The selected provider will deliver comprehensive community living supports, encompassing assistance with daily living skills, social integration, and individualized care plans.
