Bidders are required to submit three Material Safety Data Sheets (MSDS) for each product.
Submit a completed W-9 with bid response.
Products submitted must be domestically produced in accordance with the Buy American NSLA Requirement.
All nutritional analysis information must be received by Ulster BOCES within 20 days of notice of award.
All dairy products must have at least 30 days until expiration date at time of delivery.
All bidders must be an approved USDA processor of commodity products.
To be considered for an award for a group of products, bidder must bid all items within that particular grouping.
Deliveries are not direct; product must be delivered as needed through a distributor or delivery service.
No freight charges are acceptable; all prices shall be listed as ‘delivered'.
Provide a sample of requested items.
Delivery must be between 9:00 AM and 2:00 PM on regular school days, Monday through Friday.
Suppliers will be required to give no less than one day prior notification to each school of delivery.
Shipping cartons, containers, boxes, and any other packages must be marked to the attention of the person specified on the purchase order.
Commodities shall be properly marked and securely packed.
Frozen items must be delivered in properly insulated, mechanically or thermostatically temperature controlled refrigeration units.
Brand name and pack size must be submitted.
Two complete carbon copy extended company invoices must be supplied upon delivery.
Invoices must include item quantity, product item number, item description name, case bid price, and all extensions for a completed accurate invoice with totals.
Sign the Acknowledgement of Special Instructions to Bidders and General Conditions of the Contract.
Submit Bid Form, Bid Proposal Certification, Non-Collusive Bidding Certification, and Iran Divestment Act certification.
