A California organization is seeking proposals from qualified vendors to provide a cloud-based SAFE system platform. The platform should feature a secure, user-friendly, and highly configurable infrastructure that facilitates structured root-cause problem-solving, transparent communication, and shared organizational learning.

Key requirements include the ability to support the reporting and management of various improvement opportunities, such as harm and risk-of-harm incidents as well as staff-generated ideas, while ensuring sensitive information is protected and all regulatory requirements are met. The system must offer configurable workflows, allowing notifications and engagement of stakeholders according to role, severity, or situation, and must ensure transparency and traceability when improvement opportunities are reassigned or transferred.

The platform should support a wide range of submissions, including problems, ideas, and grievances, with options for anonymity. It must incorporate Lean problem-solving tools, such as root cause analysis, visual management, and structured learning cycles. Additionally, the system should provide clear tracking and visualization of key metrics—such as participation, timeliness, rigor of problem-solving, learning, and organizational impact—with robust options for custom data exports. The contract duration will be three years.
