A government authority in Pierre, South Dakota seeks a vendor to provide Chronic Disease Self-Management (CDSM) programming services across rural communities in the state. The scope includes maintaining licensure for an evidence-based CDSM program during the contract and delivering lay leader trainings to support program delivery.
Vendors are expected to plan and execute CDSM workshops in rural communities, offering technical assistance throughout the initiative. The successful proposer will also be responsible for developing and implementing a marketing plan to encourage local participation, with a particular emphasis on engaging rural populations.
