Must be an eligible applicant per 7 AAC 78.030 (state agencies, political subdivisions, nonprofits, or Alaska Native entities)
Must have a current State of Alaska Business License number
Must have a current governing board roster with contact information
Must have a 15.00% matching funds contribution
Project Director must have a bachelor's degree in social work or related field with one year of experience, or three years of experience as a CAC Project Director
Must have a current federally approved Indirect Cost Rate Agreement or qualify for the 15% De Minimis rate
Must have a Medicaid Provider Number or apply to obtain one
Must have a written plan for emergency response and recovery
Must submit MDT protocols signed and dated by all involved agencies
Must participate in monthly teleconferences and an annual two-to-three-day CAC grantee meeting
Must use the Outcomes Measurement System (OMS)
Must be able to provide access to aggregate data from NCAtrak and OMS
Must not be debarred, suspended, or excluded from receiving federal or state grant assistance
