The program will include case management services that respect the cultural needs and values of clients, with a full-time equivalent (FTE) staff member serving as the primary bilingual coordinator case manager.
Key responsibilities include supporting clients in understanding their eligibility for school-based disability services, conducting case management tasks, monitoring activities using anonymous client surveys, and tracking programmatic outputs through quarterly data reports. Additionally, the grantee will be expected to recruit clients and conduct a minimum of four community outreach events annually, ensuring at least one event per quarter to promote the program and available services.
