Submit all exceptions on the Deviations and Exceptions page
List all proposed subcontractors and their responsibilities
Acknowledge all Solicitation Amendments
Disclose if debarred, suspended, or otherwise lawfully precluded from public procurement activity
Ensure all prices are F.O.B. Destination and include delivery and unloading
Submit proposal in a sealed envelope/package clearly identifying contents and solicitation number
Do not submit emailed or faxed responses
Maintain comprehensive general liability insurance ($1,000,000 per occurrence, $2,000,000 aggregate) naming Gadsden Elementary School District #32 and Yuma Educational Purchasing Association as additional insureds
Maintain Worker's Compensation and Employer's Liability Insurance as required by law
Maintain automobile liability insurance
Maintain all current federal, state, and local licenses and permits
Comply with all specific job safety requirements and occupational safety and health standards
Ensure no employee or subcontractor registered as a sex offender performs work on District premises when students are present
Comply with the Patient Protection and Affordable Care Act
Disclose lobbying activities if the contract exceeds $100,000
Submit a certification regarding lobbying
Certify independent price determination
Comply with Federal civil rights law and USDA civil rights regulations
Comply with Clean Air Act, Clean Water Act, and Environmental Protection Agency Regulations
Comply with the Contract Work Hours and Safety Standard Act if the contract exceeds $100,000
Certify not debarred, suspended, or ineligible for federal assistance programs if the contract exceeds $25,000
Comply with the Energy Policy and Conservation Act

Maintain books and records for five years, subject to inspection and audit

Submit four copies (one original, three copies) and one thumb drive of the proposal
Audit firm must be independent and properly licensed
Audit firm's professional staff must have received required continuing professional education within the preceding two years
Audit firm must submit its most recent external quality control review report and have a record of quality audit work
Auditors must be properly licensed certified public accountants or persons working for a licensed certified public accounting firm according to GAS
Audit firm must meet the independence requirements of GAS and GASB 34 & Newer
Audit firm must not have a record of substandard audit work for the last five years
Audit firm must have an external quality control review performed at least every three years in accordance with GAS
Auditors responsible for planning, directing, conducting, or reporting on GAS audits must complete at least 80 hours of continuing education every two years, with at least 20 hours in any one year, and at least 24 hours in governmental environment/auditing
Perform an annual financial audit in accordance with U.S. generally accepted auditing standards, Government Auditing Standards (GAS), and Uniform Guidance
Issue reports required by those standards and the Uniform Guidance
Complete a Uniform System of Financial Records (USFR) Compliance Questionnaire
Cost proposal must be submitted in a separate sealed envelope
Cost proposal should include separate costs for audits of basic financial statements, ACFR statements, federal programs, and USFR CQ
Submit one electronic copy (PDF) of the audit report, management letter, and USFR Compliance Questionnaire to the audit liaisons for review
Submit three paper copies and one electronic copy (PDF) of the final reports, management letter, and USFR Compliance Questionnaire to the District
Submit electronic copies of the audit reports, management letter, and USFR Compliance Questionnaire to the Office of the Auditor General and ADE's Grant Management Division
Submit one copy of the audit reporting package and data collection form to the Federal Audit Clearinghouse
Submit one copy of the audit reporting package to other pass-through entities if findings relate to federal awards they provided
Audit firm must be available to participate in one or more exit conferences
Retain audit documentation for five years

Governing Board approval for this contract is expected in April 2026.

Initial contract term is from July 1, 2025, through June 30, 2026.

Audit work must be completed by November 20.

Preliminary draft of reports and an exit conference must be held no later than December 15.

Final reports must be submitted to the District no later than December 31.
